Digital experiences of Fracture Liaison Services (FLS) around the world

Background: Effective secondary fracture prevention requires Fracture Liaison Services (FLS) to aggregate complex patient data, including patient identification, risk assessments, treatment and monitoring. While Key Performance Indicators (KPIs) exist for clinical quality, there are currently no standardised digital tools for capturing this data, and the role of Electronic Health Records (EHRs) in FLS remains poorly understood.

Methods: A literature review analysed the different assessment criteria for digital health records usability. The review domains were used to develop an online survey for FLS practitioners, which enabled us to capture their digital experiences, their requirements and helped us identify the key features needed for an effective local FLS database. The survey was circulated internationally to FLS networks. Internal consistency was verified using Cronbachs alpha.

Results: The survey received 48 responses from 13 countries (50% nurses, 27% administrators, 23% consultants). Results indicated that 36% of practitioners felt their current systems did not enhance productivity. The top four most time-consuming tasks identified were monitoring, patient identification, assessment, and service improvement. Widely reported missing features included:

Interoperability (connecting with other systems and pulling in lab and DXA results). Automated monitoring reminders (at 16 and 52 weeks). The ability to email or SMS text patients directly. Generating custom letters and tracking patients with incomplete care pathways.

Conclusion: There is a clear need to digitally optimise FLS processes. Practitioners have identified a specific set of required features for an "ideal" database, emphasising data accuracy, fast loading times, and interoperability. Implementing standardised digital tools could significantly reduce administrative burdens and improve the efficiency of secondary fracture prevention.
